Recruiting guide
Recruiting a nephrologist.
A nephrologist's week is split among hospital consults, an outpatient kidney clinic and rounds at dialysis units, and much of a group's income runs through those units. Fellowship classes are small, so groups recruit hard for each fellow.

What does a nephrologist earn?
No federal pay series for nephrology.
The BLS Occupational Employment and Wage Statistics survey publishes no occupation for nephrology. BLS’s list of job titles places the nephrologist in Physicians, All Other (SOC 29-1229). That group’s figures cover every specialty counted in it, so this guide does not print them as the pay of a nephrologist; the Physicians, All Other salary page has them by state.
Sources: BLS: SOC 2018 direct match title file, read October 25, 2026; 2018 Standard Occupational Classification: major groups and occupation definitions, U.S. Bureau of Labor Statistics.
Nephrology has no occupation of its own in the BLS survey; nephrologists are part of the much larger group described above. A nephrology offer has to be judged against other offers in the field, not against that group's figures.
Pay is commonly a salary on a partnership track, with income later coming from professional fees for clinic, hospital and dialysis patients, and from medical directorships of dialysis facilities. Candidates ask how many dialysis shifts they will round on and how directorships are shared.
Where do nephrologists look for jobs?
- Where new hires come from
- Most new hires finish a fellowship after residency
- Who employs them
- Independent nephrology groups, hospital-employed nephrology services, academic medical centers, and groups affiliated with dialysis providers.
- Who you compete with for candidates
- Academic centers with transplant and research roles, hospital employment with no dialysis rounding, and groups offering earlier partnership.
Nephrology fellows train after internal medicine residency, and the field's fellowship positions are not always filled, so each graduating fellow has choices. Groups that host fellows for electives or teach in the program see candidates early.
Practicing nephrologists move for call, for transplant work, or when a group's dialysis relationships change. A posting that spells out the hospital census, the number of dialysis units covered and the call frequency gives them what they need.

What does an open nephrology position cost?
The worked example
The recruitment cost calculator’s own example with nephrology chosen: a position open 120 days, a locum covering 60 of them at 40 hours a week, one posting on each of CareerMD, DocCafe, NEJM CareerCenter and PracticeMatch for the whole time, and a contingency agency as well.
- Revenue the empty schedule does not bring in
- $294,092 (estimate from AMN Healthcare (Merritt Hawkins), 2019)
- Locum cover, physician pay only
- $60,000 to $77,143 (estimate from Locumstory (CHG Healthcare), 2025)
- Agency fee
- Needs your first-year pay
- Job board spend
- $4,296, plus a quote
- Total, boards and agency
- $358,388 to $375,531 (estimate from AMN Healthcare (Merritt Hawkins), 2019; Locumstory (CHG Healthcare), 2025; MedStaff Nationwide, 2026)
The total leaves out one board priced by quote and the agency fee: the calculator has no published figure there for nephrology, so it asks for yours.
Figures from the recruitment cost calculator, with each published figure’s publisher and year beside it; the days, the locum cover and the boards are the calculator’s example choices, not a recommendation.
What the calculator has for nephrology
- Revenue one physician brings in: $1,789,062 a year (estimate, AMN Healthcare (Merritt Hawkins), 2019)
- Locum pay: $175 to $225 an hour (estimate, Locumstory (CHG Healthcare), 2025)
- First-year pay: no published figure, so you supply the first-year pay you expect to offer
- Contingency agency fee: 20 to 30% of first-year pay (estimate, MedStaff Nationwide, 2026)
The calculator holds $1,789,062 (estimate, AMN Healthcare (Merritt Hawkins), 2019) a year to an affiliated hospital per nephrologist, a survey figure that mostly reflects hospital admissions and procedures. Dialysis patients still need monthly visits from a nephrologist, so a vacancy shifts that rounding onto the rest of the group.
Locum nephrologists are paid $175 to $225 (estimate, Locumstory (CHG Healthcare), 2025) an hour, and agencies can supply them for hospital consults more readily than for the dialysis relationships a permanent partner builds.
Which recruiting channels work for nephrology?
Because fellows have options, groups win them on workload and partnership terms, written down clearly enough to compare.

The channels that fit, compared
| Channel | What it costs | What to decide |
|---|---|---|
| Fellowship program relationships | Teaching time, electives for fellows; no fee | Whether a partner can teach in the fellowship program, which keeps the group in front of fellows. |
| Locum cover | $175 to $225 an hour paid to the physician (estimate, Locumstory (CHG Healthcare), 2025), plus the agency's margin | Whether a locum can take hospital consults so partners keep up with dialysis rounds. |
| General physician boards | DocCafe: Quote on request | Whether a broad board adds candidates beyond fellowship programs. |
| Contingency search firm | 20 to 30% of first-year pay (estimate, MedStaff Nationwide, 2026) | Whether an experienced nephrologist for a rural area justifies the fee. |
Questions to ask
- How many dialysis units and patients would the new nephrologist round on?
- How are dialysis medical directorships assigned and paid?
- What is the typical hospital census, and how is call shared?
- What first-year pay will you offer?
Which rules bite nephrology recruiting?
Dialysis directorships and joint ventures are where nephrology meets the referral laws, because nephrologists decide where their patients receive dialysis.
Every federal rule this site cites, grouped by what a practice or recruiter is doing, is on the federal rules page. For the contract itself (pay models, tail coverage, restrictive covenants), read the physician employment contract, clause by clause. These notes describe the rules and cite them; they are not legal advice for a particular offer or contract.
Questions recruiters ask about nephrologists.
How long does it take to recruit a nephrologist?
For a fellow, plan around the final fellowship year. In regions far from training programs, a search can run much longer, and locum coverage or a telenephrology arrangement fills the gap.
What does a nephrology recruiter charge?
A recruiter on contingency charges a percentage of first-year pay; the calculator's range is 20 to 30% (estimate, MedStaff Nationwide, 2026) of first-year pay. With no federal pay series to lean on, ask the recruiter what compensation data the fee and offer rest on.
Where do nephrologists look for jobs?
Through fellowship programs and colleagues, and on the general boards listed in the job board directory.
