What does an open physician position cost?

Pick the specialty, how long the position stays open and how you plan to fill it. The calculator lines up the revenue the empty schedule does not bring in, locum cover, an agency fee and job board spend, route by route, each from a dated source.

Your figures

Calendar days, from the day the last physician leaves to the day the new one sees patients.

Set the assignment’s own schedule; the box holds the worked example’s figure.

How you plan to fill it
Job boards to post on

One posting on each, kept up for the days open. Boards limited to one specialty show only for it.

Your own figures (optional)

Each replaces the published figure; leave a field blank to keep it.

How each line was worked out

  • Days. 120 days open, 60 days covered by a locum, so 60 days with an empty schedule. The same days apply to every route; if you expect one route to fill the position sooner, change the days and compare.
  • Revenue. $2,111,931 (estimate, AMN Healthcare (Merritt Hawkins), 2019) a year, divided by 365 days, is $5,786 a day; times 60 empty days is $347,167 (estimate from AMN Healthcare (Merritt Hawkins), 2019). The survey counts the net inpatient and outpatient revenue a full-time physician brings the affiliated hospital, which is more than a practice collects on its own; a practice that is not tied to a hospital should enter its own figure.
  • Locum cover. 60 days at 40 hours a week is 342.9 hours, at $120 to $135 an hour (estimate, Locumstory (CHG Healthcare), 2025): $41,143 to $46,286 (estimate from Locumstory (CHG Healthcare), 2025). That is what the agency pays the physician; the facility pays the agency more (its margin, travel, housing and malpractice), and no agency publishes that rate, so treat this line as the floor.
  • Agency fee. 20 to 30% (estimate, MedStaff Nationwide, 2026) of first-year pay of $255,820 (U.S. Bureau of Labor Statistics, 2025): $51,164 to $76,746 (estimate from MedStaff Nationwide, 2026). The pay figure is the national mean wage for the occupation; the offer for your position may be well above or below it, so enter it if you know it.
  • Job boards. One posting on each board you picked, kept up for the 120 days, at its published price: $4,296.
    • CareerMD: free, posting is free.
    • DocCafe: ask for a quote, left out of the sum.
    • NEJM CareerCenter: $3,100, 4 postings of 30 days at $775. The board sells 30-day postings only, so the days are rounded up to whole postings.
    • PracticeMatch: $1,196, 4 postings of 30 days at $299. The board sells 30-day postings only, so the days are rounded up to whole postings.

How the numbers work.

  1. Count the empty days

The days the position stays open, less the days a locum covers. The days are calendar days, so the yearly revenue figure is divided by 365.

  2. Price the revenue the empty schedule does not bring in

Annual revenue per physician for the specialty, divided by 365, times the empty days. The published figure is an estimate from a survey of hospital finance officers; your own figure replaces it.

  3. Price the locum cover

Locum days times the hours worked per week divided by seven, times the published hourly pay range. That is what the physician is paid; the agency bills the facility more, so this line is a floor.

  4. Price the routes

An agency fee is a percentage of first-year pay. Board spend is one posting on each board you pick, kept up for the days open, at its published price; a board that only quotes is left out of the sum and named.

  5. Add them up, route by route

Job boards: lost revenue, locum cover and board spend. Agency: lost revenue, locum cover and the fee. Both: all four. Ranges stay ranges, from the low end of every range to the high end.

Data and assumptions

Every figure the calculator brings to the page, with its publisher, the year of the edition, the page it was read on and the day. Estimates are survey averages or published typical ranges and say so wherever they appear. Your own figures replace any of them.

Revenue per physician

Average net inpatient and outpatient revenue one full-time physician brings the affiliated hospital in a year, by specialty, from the latest public edition of a survey of hospital finance officers. It is revenue to a hospital, not a practice’s collections.

Locum pay

Hourly pay for locum physicians as Locumstory, published by CHG Healthcare, shows it. It is what the physician is paid, not what the facility is billed; the agency’s margin, travel, housing and malpractice come on top and are quoted per assignment.

Assumptions

  • Days open are calendar days, so the yearly revenue figure is divided by 365.
  • A day a locum covers is not an empty day; locum hours are the days covered times the weekly hours divided by seven.
  • Every route uses the same days open. If one route would fill the position sooner, change the days and compare.
  • Board spend is one posting per board for the days open, at the board’s published single-posting price; packs and discounts are not applied.
  • The agency fee is the percentage range times first-year pay, owed once on hire.

Questions about this calculator.

How much does it cost to recruit a physician?

It depends far more on how long the position stays open than on the ad or the agency. In the worked example above (family medicine, 120 days open, a locum for 60 of them) the routes come to job boards $392,606 to $397,748, agency $439,474 to $470,198, boards and agency $443,770 to $474,494, all estimates, and the largest single line is the revenue the empty schedule does not bring in.

Put in your own specialty, days and figures to see your case; every input shows where it came from in the data and assumptions below.

Where does the revenue figure come from?

From the latest public edition of the AMN Healthcare (Merritt Hawkins) Physician Inpatient/Outpatient Revenue Survey, which asked hospital finance officers what one full-time physician brings the affiliated hospital in a year. By specialty it runs from $1,440,217 a year for ophthalmology to $2,310,000 to $3,484,375 a year for cardiology (estimate, AMN Healthcare (Merritt Hawkins), 2019).

That is revenue to a hospital, including tests and admissions the physician orders, so it is larger than what a practice collects. The survey gives one average per specialty, so the calculator prints it as an estimate. A practice with its own numbers should use them.

What does a locum tenens physician cost?

Agencies quote facilities per assignment and none publishes those rates. What is published is what the physician is paid: Locumstory, run by CHG Healthcare, lists hourly ranges by specialty, from $100 to $120 an hour for pediatrics to $375 to $500 an hour for hematology and oncology (estimate, Locumstory (CHG Healthcare), 2025).

The facility pays the agency more than that: its margin, travel, housing and malpractice coverage come on top. The calculator uses the published pay as the floor for the locum line; enter the rate from your agency’s quote to replace it. The locum agency directory lists who staffs which specialties.

What do physician recruiters charge?

Most quote a percentage of the physician’s first-year pay. MedStaff Nationwide gives 20 to 30% as the range most commonly quoted for a contingency search, owed only when you hire (estimate, MedStaff Nationwide, 2026), and the calculator uses that range. Retained searches bill part of the fee up front, whether or not the search fills.

On the worked example’s first-year pay of $255,820, that is $51,164 to $76,746. Ask any firm what you owe if the search never fills, and get the answer in writing.

Does the calculator keep what I type?

What you type is used on this page to work out the result, and it is written into the page address so the result can be bookmarked or shared; anyone you send that address to can see the figures in it. The calculator does not send your figures to us, and nothing you enter is added to the site’s data. Like any page you load, the address reaches our host when the page is requested, as the privacy policy explains.
